By entering the name of the person submitting this ePermit application in the box above AND in lieu of signed, sworn and notarized signatures of the property owner, agent and/or contractor and under penalty of perjury, I: 

1. Understand per F.S. 489.103(7)(a)(4)(c)(2), that building permits are not required to be signed by a property owner unless he or she is responsible for the construction and is not hiring a licensed contractor to assume responsibility;
2. Understand it is the owner’s or operator’s responsibility to comply with the provisions of F.S. 469 and to notify the Department of Environmental Protection, Northeast District, located 8800 Baymeadows Way, Suite 100, Jacksonville, FL 32256 (904)256-1566 at least (10) working days prior to his or her intentions to remove asbestos, when applicable, in accordance with State and Federal Laws;
3. Certify all materials installed will match the product(s) listed above will be installed in accordance with all applicable codes and standards set forth in the FBC. A legible copy of each manufacturer’s installation instructions will be posted on-site for the inspector;
4. Certify all protected trees [or any tree retained for tree mitigation or conservation credit] will be protected prior to the issuance of a permit and/or during demolition and/or construction.  No trees will be removed, damaged or destroyed without first obtaining a permit (tree removal/site clearing permit application form and tree survey required).
5. Understand it is unlawful for any person to in any way meddle, tamper with or otherwise alter or change the utility system(s) on any premises and that the consumer or property owner may be subject to a fine if evidenced and that the City retains the right to discontinue such services until the defect is corrected;         
6. Understand that if there is an error with one of the uploaded files during processing, I may be advised to upload again or to submit in hardcopy format;
7. Understand that FAILURE TO RECORD A NOTICE OF COMMENCEMENT MAY RESULT IN THE PROPERTY OWNER PAYING TWICE FOR IMPROVEMENTS TO PROPERTY AND FURTHERMORE, SAID RECORDED NOTICE MUST BE POSTED ON THE JOB SITE BEFORE THE FIRST INSPECTION. THE OWNER SHOULD CONSULT THEIR LENDER OR AN ATTORNEY IF THEY INTEND TO OBTAIN FINANCING
8. Agree to conduct transactions with the City of Jacksonville Beach by electronic means and that the electronic signature above shall serve as my signature authorization on permit applications and/or construction documents to be used for the review, approval, and issuance of building permits electronically;
9. Understand the electronic signature and payment of required fees make this application valid and binding to the same terms, conditions, force and effect as handwritten signatures and that I may be required to provide traditional signatures at a later date; 
10. Understand that work may not begin until a permit is approved and posted on site;
11. Certify that I am either the legal property owner of record OR that I have been authorized to submit this application and perform the work described above; and 
12. Declare that I have read and examined this application and all of the foregoing information is true and correct to the best of my knowledge and belief.
