By entering the name of the person submitting this ePermit application in the box above AND in lieu of signed, sworn and notarized signatures of the property owner, agent and/or contractor and under penalty of perjury, I: 

1. Understand per F.S. 489.103(7)(a)(4)(c)(2), that building permits are not required to be signed by a property owner unless he or she is responsible for the construction and is not hiring a licensed contractor to assume responsibility;
2. Certify all materials installed will match the product(s) listed above will be installed in accordance with all applicable codes and standards set forth in the FBC. A legible copy of each manufacturer’s installation instructions will be posted on-site for the inspector;
3. Understand it is unlawful for any person to in any way meddle, tamper with or otherwise alter or change the utility system(s) on any premises and that the consumer or property owner may be subject to a fine if evidenced and that the City retains the right to discontinue such services until the defect is corrected;         
4. Understand that if there is an error with one of the uploaded files during processing, I may be advised to upload again or to submit in hardcopy format;
5. Agree to conduct transactions with the City of Jacksonville Beach by electronic means and that the electronic signature above shall serve as my signature authorization on permit applications and/or construction documents to be used for the review, approval, and issuance of building permits electronically;
6. Understand the electronic signature and payment of required fees will make this application valid and binding to the same terms, conditions, force and effect as handwritten signatures and that I may be required to provide traditional signatures at a later date; 
7. Understand that work may not begin until a permit is approved and posted on site;
8. Certify that I am either the legal property owner of record OR that I have been authorized to submit this application and perform the work described above; and 
9. Declare that I have read and examined this application and all of the foregoing information is true and correct to the best of my knowledge and belief.
